4-H Project Recognition Awards Application

The first step in applying for PROJECT MEDALS is to verify that you have met the requirements for the awards you are applying for.

Remember: It is your responsibility to be sure that you have met the criteria for the awards you are applying for. Failure to meet the requirement may result in denial of an award. If you have any questions please contact the office. You must bring to the interview a completed copy of the criteria form.  Once you have printed off and completed the criteria form you should register for the award.

DO NOT APPLY UNTIL YOU ARE SURE YOU QUALIFY

The purpose of the 4-H Project Recognition Awards are to recognize youth for their hard work, dedication and personal development that they have achieved through their involvement in their 4-H club, projects and activities.  Because youth develop and achieve in different ways, there are different levels of recognition that they can receive. 

The Personal Growth Certificate ( Originally designed for Cloverbud participants but anybody can apply for a Personal Growth Certificate.)

Personal Growth Certificate Criteria

1. Must be currently enrolled in 4-H

2. Must have completed at least 2 activities in at least one project area that the member is enrolled in.

3. Submit a reflection on what the 4-H Member learned or individual accomplishments that they achieved through their 4-H involvement.  This reflection can be in written essay or story form, a picture story along with captions and descriptions of each picture or any other means that the 4-Her would like to tell their "4-H Story"  maximum 4 pages.

The Project Medal Award  

1. Must be currently enrolled,

2. Must complete a minimum number of years in a 4-H project area.  Cloverbud experience does not count.

3. Must bring completed criteria forms to the interview and discuss goals, new things learned or tried, highlights of the year.  Animal applicants should have knowledge on animal care, management, health records, breeding/calving, diet, income and expenses.  Follow this link to print off the necessary forms http://cceoneida.com/4-h-youth-development/awards-recognition

4. Must have completed the specified number of activities for that specific project.

5. Must have a Creative Communication and logged in 6 hours of Community Service to Faces of 4-H

Project Certificate Youth must meet all the requirements of the project medal with the exception of participating in the Interview.  Completed copies of the Medal Criteria must be received by October 1st.

The Outstanding Achievement Award recognizes up to 4 youth – one from each of the following project groups, for outstanding work and accomplishments in their project(s). Emphasis is placed on growth and advancement in their project area(s), as well as personal growth in terms of leadership, community service and sharing 4-H experiences with others.                                                                        

1. Outstanding Personal Development:  Awarded 4-H Capital Days in Albany for youth 14 and older  

2. Outstanding Family & Consumer Science

3. Outstanding Animal Science      

4. Outstanding Environmental Science  

 (2,3,4 choose one of the following): 4-H Career Explorations at Cornell University - “University U” for youth entering 8th & 9th grades, “Focus for Teens” for youth entering 10th to 12th grades.  Teen Winter Weekend ages 13+, ABC Trip ages 14+, STARR ages 13+, NY State Forestry Program ages 10+). Additional youth will earn prizes and gift certificates.

INTERVIEWS WILL BE SCHEDULED FOR COLUMBUS DAY (in the morning)
